Is Gender-Affirming Hair Restoration Right For Me?

Strong candidates for NeoGraft® gender-affirming hair restoration are in good overall health and have sufficient donor hair at the back of the head to support transplantation. For transgender patients, it’s strongly recommended to be on a stable hormone replacement therapy (HRT) regimen for at least one year before undergoing FUE. This allows your hair pattern to stabilize so that Dr. Ramineni or Dr. Eidelson can design a hairline or facial hair plan that delivers predictable, lasting results. 

How to Prepare For a NeoGraft® Treatment in Washington DC

In the weeks leading up to your NeoGraft® procedure, you’ll be asked to stop taking blood-thinning medications and supplements — such as aspirin, ibuprofen, Vitamin E, and fish oil — at least 10 days before your appointment. Avoid alcohol for one week prior and stop all nicotine use at least three days before, as nicotine restricts blood flow and can compromise graft survival. On the day of your procedure, wash your hair with regular shampoo but skip all styling products, and wear a button-down shirt so nothing needs to be pulled over your head.

The Procedure

The NeoGraft® FUE procedure begins with a detailed planning session during which Dr. Ramineni or Dr. Eidelson maps your donor and recipient areas based on your personal goals. A local anesthetic is applied to the donor area — typically the back of the head — and the NeoGraft® device then uses a gentle pneumatic suction system to extract individual follicles one by one. Because the process is automated, each graft is handled consistently. Follicles spend minimal time outside the body, which supports a higher survival rate.

Once harvesting is complete, your surgeon creates tiny recipient sites in the target area, carefully considering the angle and direction of natural growth. The harvested follicles are then placed into these sites with utmost care. The full procedure is performed in a single day.

After Your Treatment

The first 72 hours of recovery are the most critical. Keep your head elevated while sleeping, avoid touching the newly placed grafts, and gently mist the treatment area with the supplied spray bottle every one to two hours to support hydration and healing. 

Beginning around day four, you can carefully rinse the area using a cup of lukewarm water and baby shampoo; avoid letting direct shower pressure contact the grafts. Strenuous exercise and heavy lifting should be avoided for at least 10-14 days.

Around weeks 2-4, the transplanted hair will shed — this is an expected part of the process. The follicles remain intact beneath the skin and will begin producing new permanent hair within 3-4 months. Significant growth and thickening become visible between months 6 and 9, with final results typically appearing at the 12-month mark.

Does the procedure hurt?

Most patients find NeoGraft® FUE very comfortable, as a local anesthetic is applied to the scalp before extraction and placement begin. Some mild soreness in the donor area may be felt in the days following the procedure, but significant pain is uncommon.

Is male or female pattern baldness possible after gender-affirming hair restoration?

The transplanted follicles themselves are resistant to pattern baldness because they are harvested from the “permanent zone” at the back of the head, where follicles are genetically programmed to resist the hormones that cause hair loss. However, any existing hair in the recipient area that was not part of the procedure can still be susceptible to male or female pattern baldness over time. This is why stable HRT and careful treatment planning are especially important for transgender patients.
